Cataract Treatment
ADVACEMENT IN CATARACT SURGERY
Micro Incision Cataract Surgery.
Cataracts are treated by surgically removing the cataract and implanting an artificial replacement called an intraocular lens (IOL). Recent advances in the cataract surgery procedure have gradually reduced the size of the incisions needed for surgery from 12mm to 3mm, making it safer and less invasive.
Micro incision cataract surgery (MICS) TM
MICS is the most advanced, state-of-the-art technique for treating cataracts.
Now only two very small incisions of 1.8 mm or less
are required to perform the entire cataract surgery procedure
MICSTM procedure
Specialised MICS equipment and instruments are used to perform the surgery in a safe and stable manner. An ultrasound probe is used to safely break up the cataract using a technique called phacoemulsification. The cataract is the artificial lens.
Bausch & Lomb’s new Stellaris™ Vision Enhancement System is coupled with an array of specialized sub-2 mm instruments — a microincision artificial intraocular lens — to allow surgeons to perform cataract surgery using smaller surgical incisions that can enhance patient comfort and recovery. To gain the full benefit of the MICS technique, it is important to also use an IOL designed to be implanted through a 1.8 mm micro incision.
The larger incisions used for standard cataract surgery and standard IOLs may cause unwanted changes to the eye, known as surgically induced astigmatism. These changes can cause image interference and reduce the quality of vision.
What are the different types of intraocular lenses implanted after cataract surgery.
As the natural lens plays a vital role in focusing light for clear vision, artificial-lens implantation at the time of cataract surgery is necessary to yield the best visual results. Because the implant is placed in or near the original position of the removed natural lens, vision can be restored, and peripheral vision, depth perception, and image size should not be affected. Artificial lenses are intended to remain permanently in place, require no maintenance or handling, and are neither felt by the patient nor noticed by others. There are a variety of intraocular lens styles available for implantation, including monofocal, toric, and multifocal intraocular lenses:-
1. Monofocal lens -
These lenses are the most commonly implanted lenses today. They have equal power in all regions of the lens and can provide high-quality vision at a single focal point (usually at distance). They usually require only a light pair of spectacles for optimal distance vision correction. However, monofocal lenses do not correct astigmatism, an irregular oblong corneal shape that can distort vision at all distances, and require corrective lenses for all near tasks, such as reading or writing.

Multifocal lens-
Multifocal intraocular lenses have a variety of regions with different power within the lens that allows individuals to see at a variety of distances, including distance, intermediate, and near. While promising, multifocal lenses are not for everyone. They can cause significantly more glare than monofocal or toric lenses. Further, multifocal lenses cannot correct astigmatism, and some patients require additional surgery such as LASIK to correct astigmatism and maximize their unaided vision.
WHAT SHOULD I EXPECT FROM A CATARACT SURGERY?
Cataract surgery is a microscopic surgery usually performed under local anesthesia. The cloudy lens is removed leaving its capsule behind, within which a permanent intraocular lens is implanted. The refractive power of the implanted lens is calculated so that there is a minimal need for post-operative spectacle prescription. However, glasses will be required for near work.
After cataract surgery, one may return almost immediately to all but the most strenuous activities. Medication must be administered as per the instructions of the surgeon. Your surgeon will tell you when you may return to work. Conventional cataract surgery, which involves removal of the lens in one piece, results in an incision size of 10-12 mm and closure of this large incision with multiple stitches. This has been replaced with the modern technique of cataract removal [phacoemulsification].
